HIPAA Privacy and Confidentiality
Living Brave Through Breast Cancer and Living Brave Home Care Services, LLC are HIPAA covered entities and are committed to protecting the privacy, confidentiality, integrity, and security of protected health information and other personal information entrusted to us.
Both organizations maintain privacy and security practices designed to comply with applicable requirements of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), including applicable Privacy, Security, and Breach Notification requirements, as well as applicable federal and State of Michigan privacy laws and regulations.
Protected health information may include information relating to an individual's health condition, healthcare, services, treatment, payment for healthcare, or other health information that identifies or could reasonably identify the individual.
We may collect protected health information and other personal information through referrals, applications, program participation, support services, service inquiries, intake forms, care coordination, caregiver communications, and other interactions with our organizations.
Protected health information will be used or disclosed only as permitted or required by applicable law. When HIPAA requires an individual's written authorization before a particular use or disclosure, we will obtain that authorization.
Living Brave Through Breast Cancer and Living Brave Home Care Services, LLC maintain administrative, physical, and technical safeguards designed to protect protected health information against unauthorized access, use, alteration, loss, or disclosure.
Although Living Brave Through Breast Cancer and Living Brave Home Care Services, LLC work together to support individuals and families affected by cancer, they remain separate legal entities. Protected health information will not be shared between the entities merely because of their relationship. Any use or disclosure between the organizations must have an appropriate legal basis under HIPAA and other applicable law.
Individuals may have rights under HIPAA concerning their protected health information, including, when applicable, the right to request access to or copies of their records, request corrections, request certain restrictions, request confidential communications, receive an accounting of certain disclosures, and receive a copy of the applicable Notice of Privacy Practices.
